Health Care Integrated Services’ Agency Service Delivery Protocol

“Safe at home, in school, and out of the juvenile justice system.”

Responsibilities to the Student

Health Care Integrated Services shall:

  • Respect each student as a unique individual and encourage the complete development of each student.
  • The Agency is concerned for the total needs of the student (health, educational, vocational, family, personal, and social).
  • Demonstrate respect, acknowledge each student as a unique individual, and emphasize a comprehensive personal therapeutic development
  • Institute appropriate referrals when the Agency cannot or no longer be of professional assistance to the student

Responsibilities to the School

Health Care Integrated Services shall:

  • Effectively utilize the abilities of teachers and other staff members to promote optimal treatment services for students.
  • Maintain a professional relationship with school personnel and other agencies in the school setting.
  • Develop efficient and responsible treatment modality procedures and policies to meet the expressed needs of the student within the guidelines of NASBHC.

Responsibilities to the Community and Parents/Guardians

Health Care Integrated Services shall:

  • Seek out and work cooperatively with agencies, places of employment, social services, the juvenile justice system, etc.
  • Maintain a professional relationship with outside agencies and other educational or community programs while working on behalf of the student.
  • Form a collaborative working group with neighborhood organizations, businesses, colleges, and universities.

Treatment and Intervention Plans for Students

Cognitive behavior therapy is the focus of an action-oriented approach. Students shall be helped to overcome maladaptive behaviors that restrict choices and learning.  Counselors shall follow approved techniques and skills of self–change so that they can improve their lives by altering one or more of the various factors influencing their behavior, including prevention, reduction, and elimination of negative influences.

Services Timetable

Approved staff must be present on certified site location as medically necessary; schedules shall be based on actual student’s participation and or site need.

Community Service Credits

With the approval of the Education sites, the students may receive up to 45 hours of community service credits to satisfy graduation requirements for participation in the life skills program and or Youth Health Educator.

Data Collection

The participating staff shall record a progress note for each student participating in any session. Progress notes are individual narrative summaries and shall include:

  1. A description of the student’s progress on the treatment plan, problems, goals, action steps, objectives, screenings, and/or referrals.
  2. Information on a student’s attendance including the date, month, day, year, and duration in minutes of individual or group sessions.

Confidentiality

The Agency shall protect the confidentiality of student records, and shall release personal data according to current Federal and Confidentiality Standards.  Health Care Integrated Services shall share information regarding a student only to those who are properly authorized to receive information via conformed consent and adhere to all applicable state and federal laws and rules. Counselors and professional staff shall report information only under the following conditions:

  • When students pose a danger to themselves or others
  • When the staff believes that the student has been a victim of incest, rape, child abuse/, or some other crime
  • When the staff determines that the student needs hospitalization
  • When information is made an issue in a court action

The staff’s primary obligation shall be to protect the student’s disclosures as a vital part of the therapeutic relationship. Staff shall inform the student about conditions that limit confidentiality and the duty to warn when imminent danger is present.

Safety Procedures

All Designees of Health Care Integrated Services shall promote safety abiding by Districts policies and procedures in effect. Participate in the school’s emergency drills.  I.D. Badges shall be visible and worn at all times. In the event of a Disaster/Crisis, the School emergency response plan shall be followed, with no exceptions.  The principal or designee shall be notified immediately to warrant any reportable circumstance.  Written documentation shall be submitted to school officials for reportable unusual occurrences or incidents within 24 hours of the event. Any disruptions or safety concerns shall be reported immediately following school and district protocol. Documentation of all unusual occurrences shall be submitted to school officials within 24 hours following the incident.  Likewise, any disruptions or out-of-order conduct by Health Care Integrated Services personnel shall be reported to the President and or Director of services immediately.  Any complaints arising from Health Care personnel shall result in immediate removal from the site as well as pending investigation. Serious violations shall result in local law enforcement reporting and termination.

Code of Ethics

It is the policy of this Agency to abide by the Code of Ethics for all medical professionals, certified behavioral health counselors, and other health professionals.

Non-Discrimination

The Agency shall prohibit discrimination against students, the public, or others based on race, color religion, national origin, age, gender, sexual orientation, marital status, disability, or economic conditions.

Competence

Competent professional service requires thorough knowledge of medical, substance abuse, and or mental health, skills in presentation and service techniques, the preparation necessary to assure the highest level of quality service, and a willingness to maintain current and relevant knowledge through ongoing professional education, staff development, and ongoing in-service training. The Agency shall not operate beyond the level of skill or training for any professional discipline.

Professional Standards

The Agency shall maintain the highest professional standards and shall not:

  • Directly or by implication claim personal knowledge, qualifications, or affiliations that the staff does not possess.
  • Misrepresent their certification to the schools.
  • Engage in false or misleading communication about their own or others professional
    abilities

Sexual Harassment Policy

Sexual Harassment is prohibited by Health Care Integrated Services, Medical, Mental Health, and Substance Abuse Services, and by state/federal law. All allegations shall be treated seriously. No staff may frighten, insinuate either explicitly or implicitly, that a student’s submission to or rejection of sexual interactions will have an effect on medical, substance abuse, or mental health treatment. Once such an allegation is reported, the site manager shall contact the school district, any outside agency involved, and the appropriate school official (Principal).  Complaints will be kept as confidential as possible, and all measurements will be taken to provide protection from retaliation and to stop further harassment, including immediate removal from the site should any Health Care staff be involved. Should an investigation determine that the allegations have merit involving health Care staff; the staff will be held liable for their actions and will be terminated as well as report to local law enforcement when warranted. Precise Observation and measurement of behavior shall be used to help our staff determine the best way to increase positive behavior at all times. In addition, observation and measurement shall be used to determine a particular intervention effect. This monitoring shall be used to provide guidelines for the continuation or the modification of the current intervention, techniques utilize, systematic assessments, and measurements to provide accurate information about student progress and the successes of the program.

Admission Criteria and Procedures

Health Care Integrated Services shall perform all of the following when medically required: Acquire parent or minor consent. Complete a personal intake, medical screening, ace screen, substance history for each beneficiary upon enrollment to the health program. Complete an assessment of the physical condition of the beneficiary (a) A physical examination of the beneficiary may be performed by the agency’s physician, registered nurse practitioner, or physician assistant authorized by state law to performed such prescribed procedures.

Service Fees

The service fees shall be reimbursed by CHDP, CHiP, or Medi-Cal if applicable, notwithstanding Minor Consent. There shall be no fee to the Students Parents/Guardians or school district. Access to services shall be afforded to all
students, regardless of ability to pay.

Minor Consent

Parent permission and consent are required for all children under 12 years of age. Family Law Regulates a child 12-17 may consent to certain treatment per family code 6925 as long as a basic level of comprehension is displayed Students who are under the age of 12 need parental consent to receive screenings, counseling, and substance /mental health and ace screen services. Health Care Integrated Services Respects the right and responsibility of parents to assist their children in decision–making. Parents are informed about the role of the counselor and the confidential nature between the counselor and the patient.

Students 12 years and over may request services without parental permission under the Minor Consent Law, California Family Code Par.6924.

Point of Entry

All School Districts Policies and Protocols. Orientation of SBHS to all available District Staff. Training regarding “How to recognized signs of Behavioral Health issues”

Referrals

Referrals are accepted from School administrators, School Nurse, Teachers, Parents, Student Self Referrals, and Outside Approved Sources.

Medical Screenings, ACE, Substance, Behavioral Health Screen and Referral

Each School Shall designate a contact person, the Site Coordinator responsible for the distribution, collection, and confidential faxing of completed referral forms to a primary location for a clinical staff review and to ensure an appointment is made for the initial assessment within three to five days notwithstanding crisis referrals, which shall be seen within 24 hours. Field Trip Permission Slip

All field trips and off-site activities shall require parental permission and release of liability for Health event and other.

Health Care Permission Slip

After the initial appointment is set for an assessment and or any follow-up services including groups, a Health Care Integrated Services Permission Slip shall be completed by the agency and the student will be excused from his or her elective classes for services upon approval of school personnel. To make certain that students skip regularly required class participation, health care personnel shall escort the student back to class or call to confirm students have reported to the proper course.
