when approaching to Alex i would introduce myself and tell him what am going to do for his visit, usually telling the patient what you doing to them before makes the patient less stress with as well asking everyday questions (how hes doing in school and have he play any sports)
t0 make alex stay at ease i would do small talk during the check up like what he likes to play and ask open questions like what color you liked/ what you do in your freetime
