You are part of a healthcare team working in a pediatric clinic. It’s nearing the end of the day, and you encounter a situation where a child arrives with a potentially urgent medical issue just before closing time. The child’s parent seems anxious, and you need to decide how to handle the situation within the clinic’s closing procedures.

– Hello my name is Emanuel Tejada(Leah Group) and I will be your medical assistant for today. I will be taking your vitals, and quickly transfer you to the doctor.

When there are urgent care cases when the clinic is starting the closing procedures as the medical assistant the first and most important step is to provide services and help to the patient, its difficult especially if its during closing time but that’s why every member should be prepared if its severe cases provide first aid and check vitals to make a smooth run even though its closing time. It’s also best to not be upset or mad because you want to go home and be done with closing the facility. This could overwhelm yourself, the staff and the patient helping the patient is far more important than closing the clinic quickly.

I’ll start by greeting and introducing myself with a calm demeanor so they don’t get scared or nervous

– Outline how you would coordinate with other staff members, including receptionists, nurses, and physicians, to manage the urgent case effectively.

I would coordinate with other staff members to inform them that this case is special. To the receptionist I would let them know to quickly attend this case, or skip it to take the kid directly to the ER. I would inform the nurses and physicians that this family is really anxious. To minimize the stress I would have recommended to these medical staff to be polite and careful of the word usage. Even though the clinic is in a closing procedure, this is an urgent case. Therefore, as medical professionals it would be the best if we attend this family. If we are missing supplies or have tired personnel we could quickly transfer this patient to another open clinic or hospital.

Some ethical dilemmas that could arise during the process of this event is that while still taking care of the patient Medical assistants or staff members still do closing procedures, this could overwhelm the MA because they might feel like they have too much work and cause even more problems in the setting. Instead of overwhelming yourself and trying to do this individually, ask for assistance and support so the procedure is thorough and well taken care of.

The importance of documentation is that we don’t know when or how a problem will come and by documentation we know what the patient went through and when they did. Of course legal protection is important but having patient safety and privacy is more important.

This patient needs urgent care, but since the clinic is in a closing procedure we needed to quickly determine if we could help the patient out. If the clinic still has supplies and the personnel quickly agree we can proceed to attend this patient. However, if we don’t have the necessary supplies or some personnel left we should transfer the patient to another clinic or hospital.

Managing cases during closing procedures is important because there is either two options. In these two options you need to quickly assess the situation because this is an urgent case. We either asses if we can attend this patient or send them to another clinic or hospital. Depending on the personnel and resources needed we can quickly decide on the the path taken.

Before administering the vaccine, I would tell them the importance of each vaccine. For example, if I were to administer “Polio”, I would tell them the importance of this vaccine.

After that I would try to to collaborate with other medical staff, and tell them of any suspicious of which vaccine caused this.

I would communicate the purpose of emergency equipment, which is to help fight the allergic reaction. Since the parents are kind of skeptical already, I would inform them that this is a common medication to treat adverse allergic reactions. Hopefully they understand and they do not enter in a passive aggressive manner or tone.

A 5-year-old child is brought to the clinic by their parents with symptoms of vomiting, diarrhea, and lethargy over the past 24 hours.

Initial Assessment:
– Describe the steps you would take to gather information about the child’s medical history, the onset of symptoms, and any contributing factors.

If the patient’s medical history is not present in the EHR, we could inquire the patient or the parents about the child’s previous medical history. We would ask for any previous conditions, surgeries, diseases, and medications of the child.

The onset of symptoms could be assessed by the status of the urgency level. In this scenario and based on the symptoms, the child is in an urgent care situation.

Prioritization:
– Based on the symptoms presented, determine the urgency of the child’s condition using triage protocols. Explain why you categorized the patient as emergent, urgent, or non-urgent.

Based on the symptoms presented we would classify the child’s condition as urgent. Dehydration can cause damage to the kidneys, heart, and brain. Even though the symptoms don’t seem harmful, it is important to treat them quickly. However, compared to someone who suffered an open wound from a gunshot or stab, these people would have to be attended first due to the dangers of infection or blood loss. Therefore, urgent care is the best placement for the child, since the severity level is not the highest in the triage protocol.

Dehydration can lead to death if not treated immediately. Some of the symptoms include confusion, dark urine, and no urine.

A vital sign that we could use to assess the hydration status of the child would be by testing their conscious and confusion levels. If the child is completely passed out it is safe to assume that they are in a critical dehydrated situation. If the child is confused, we can assume that the children might be dehydrated to a midpoint critical situation.

Since the child is vomiting and has diarrhea, we could use this information to assume that the child has lost more minerals and fluids. This means that the child might need urgent care, so that we could help replenish these lost fluids and minerals.

An immediate intervention that may be necessary for the child would be taking them to the ER to restore the child’s body fluid balance as quickly as possible. The rationale behind this is to keep rehydrated the child, and keep them safe!

We would respectfully communicate with the parents, and inform them that the children isn’t in a critical condition(death situation). However, for the safety of the child, we would advise the parents to pay more attention to the hydration levels of the child.

Furthermore, we could advise the parents to buy “Electrolit” to keep the child hydrated. We could potentially give the parents a pamphlet informing them over how to avoid this situation.

1: What challenges did you encounter during the simulation, and how did you overcome them?
Some challenges that I encountered during the simulation were that the clinic was unexpectedly busy, hence all the systems and equipment ran out. Therefore, I had quickly restock the supplies to resume the care towards the patient.
2: How does each role contribute to the overall efficiency and quality of patient care?
The medical assistant contributes to the overall efficiency and quality of patient care by performing vitals, closing/opening the office, updating information and appointments in PM/EHR, cleaning the exam room and answering patient calls. Other roles contribute by keeping confidentiality and primal care to the patient.
3:Why is compliance with HIPAA regulations critical in managing patient records?
It is important to keep compliance with HIPAA regulations in managing patient records to keep the files safe or private from other people and ordered into the correct place. These regulations keep the patient files safe and sound!
4:How do efficient procedures and the use of technology enhance patient-centered care in pediatric healthcare settings?
The use of technology like the PM/EHR or the automatic blood pressure tools can fasten the process of the care to the patient in an efficient way. Similarly, the use of technology can help when the clinic is busy, since it can fasten the process of care in this ambulatory setting.

One of the main challenges in this scenario are not being prepare and not having the necessary resources.
– Discuss how these tasks and challenges impact the readiness, safety, and efficiency of the clinic.
One way to conduct a detailed facility inspection is by reviewing all the regulatory standards, and making sure that they are met at the clinic. Stocking on supplies, setting up adequate furniture at the waiting room, having an operational AC, proper waste management, and having competent workers should be the first priority. Since it is a pediatric clinic we can focus on decorating with paintings, having a playground section, and having cartoon drawings on walls.
– Propose practical solutions and strategies to address these tasks and challenges.
– Consider the roles of healthcare providers, administrative staff, and other team members in implementing these solutions.
To ensure that the clinic is safe and ready for patients I would examine the area and check the check in list where my co-workers show if they clean that area and if not, I would do it without hesitating because the safety of our patient is our responsibility just like our safety also this helps keep our job and our well beings safe. The purpose of keeping our safety regulation standards safe is to prevent any harmful material in areas that can cause fetal harm to us and patients so for that reason the steps that involved inspection and initiative should be a proper solution to address this situation this will help everyone with keeping us safe and strong.

Having emergency exits and ramps is a must. A potential problem that I have identified is having unprepared staff. To solve this problem, we can have practices and meetings to prepare for the grand opening.

It is obvious that the patient in a clinical or hospital setting should be the first priority. Therefore, we need to focus on renewing the disposal system. It is proven that an outdated disposal system can lead towards unhygienic and unsafe practices that lead to disease spreading, accidents, and even death.
– Discuss how these issues impact the safety, comfort, and well-being of young patients and their families.
The impact of the poor waste management can cause very serious and dangerous actions it can harm or transmit germs that can be deadly to the patients and clinical assistance to keep the patient safe and everyone that surrounds the area it’s better to have proper hygiene and have a cycle that has everything in order like a new waste management that will keep things separate such as hazard materials in one disposal and recycle practices in other also waste like trash material in another disposal it’s more safer this way because if keeps everything together.

Our solution to this problem would be to buy designated bins and trash cans towards certain purposes. For example, we could buy more bins for biohazards and sharps. The bins or trash can be reused. To add on we could do the same by adding recycling, trash, and plant material bins. Furthermore, we could buy trash containers that could be designated to have certain purposes. These containers could be put in the parking lot, and this is where trash bags could be thrown away.

It can be pretty challenging for the people that collect the trash and having everything separate will help you as a medical assistant and the community towards a better environment.

A potential problem that may affect this clinic is not having enough funds to buy the required equipment. Another concern is that patients may not follow the rules to separate trash in their designated bin. Some solutions for these problems are taking loans to have funds for our equipment such as bins, trash cans and containers. The solution for the other problem would be to advertise the idea of putting trash in the correct place to the patient.

I agree with you, Apple, increasing technology in the medical field can enhance the care to patients. However, being unprepared or undertrained can be potential risk. Also, every medical assistant should know how to file vitals and patient information on paper in case of unexpected events like power outages.
